Images not showing up

I can’t get the images to show up in the reviews. I fill out the form, adding an image, and I go to approve it, and the dashboard shows that it has an image; but when I go to edit the review, no image appears. When I approve it, the review comment is displayed, but the image is not; and the review on the dashboard now shows up as not having an image:

On the other hand, I have managed to approve two reviews with images (I don’t know how) and they are displayed in the shortcode [site_reviews_images], but they do not appear next to the relevant review in [site_reviews]. Is this normal?
