Block comments: Add comment indicators in the block toolbar and block list #71243

What problem does this address?
One early piece of feedback that we've gotten on the experimental block level commenting feature (#60622) is that the editor should have better indicators when a comment has been left on a post.
Ex. @sethrubenstein, in this comment:
Indicating in the block list view when a block has a comment, ideally with a bubble showing how many.
In the block toolbar it would be nice to also have a bubble on the comment icon indicating how many open comments there are for that specific block
What is your proposed solution?
@mtias referenced an earlier design iteration which shows comment indicators in the block toolbar.
There is also a suggestion to show a badge in the block list view next to any blocks that have comments.
Given how closely tied these two ideas are, I've captured them both in this same issue, but this could be handled in separate PRs.
